How we checked

We started from each department's own website. On October 20, 2026 a script sent one request per department site, with a user agent that identifies the project, after reading the site's robots.txt and waiting between requests, and recorded the HTTP status. It does not re-read page content. If a site refused the request, did not provide a usable robots.txt, or otherwise signalled that automated checks are not wanted, we stopped and marked the state not re-verified (AK, AZ, CO, KS, LA, ME, MA, MI, MN, MO, NH, OH, TN, TX, VA); we did not try another way to check. Sites that signalled on October 18 that they do not want automated checks were not asked again. Open any link to confirm it, and if one is stale, email [email protected] (see corrections). Last-checked dates in the table show the day of the status check.

External review column: "Federal process" means the state has no external review process meeting federal standards, so insurers use the federal process (Alabama, Florida, Georgia, Texas, Wisconsin per CMS and HealthCare.gov). "State process" means the state runs one, per KFF's state table. Self-funded employer plans have different rules; see ERISA vs state-regulated plans.

Territories (Puerto Rico, Guam, U.S. Virgin Islands, American Samoa, Northern Mariana Islands) are not listed yet; the NAIC directory covers them.

Nothing here is a recommendation of any department over another, and we do not endorse any insurer.
